Reason The Test Or Operation Was Carried Out:
Ph Study - to test ph (acid levels) immediately above the stomach. On meds to see if sufficient or effective (in EGID many reflux meds make little difference) or off meds to help diagnose.
What Was Your Experience Of The Test/Operation:
A small tube with a thin wire inside and a probe at the end is passed down your child's nose, down the back of the throat and left at the top of the stomach. An X Ray is taken to confirm the tube is int he correct place, and that it has not gone down the trachea instead of the oesophagus. It is uncomfortable having the tube assed and it is often done under anaesthetic in conjunction with an endoscopic scope to check the state of the oesophagus and to take biopsies to ascertain whether there is any cell level change due to reflux. Once correctly placed the tube is taped to the child's cheek, the other end is attached to a small black box and you are free to go and return in 24 hours - although some hospitals keep you inpatient overnight. There are several buttons on the box and you have to press the correct ones to "tell" the box and record when you are lying/standing or sitting, eating or drinking and then finished eating and drinking. Sometimes you are asked to mae notes about behaviours (coughing, choking etc) and sometimes there is a button to press for these. We have had many done, some placed under anaesthetic and some not. On one occasion my daughter's hands were gently bound for the first two hours because she ripped the probe tube out first time. It is not pleasant but is immediately forgotten about once the tube is removed the next day. The test can be done remotely where a small clip is left in place during the scope procedure (so can only be done under anaesthetic) and the box carried close by for 24 hours, recording is the same. We found this less reliable and the clip irritated so our daughter ate nothing and skewed the results. The clip detaches after 48 hours and is passed normally in the stool -you can rarely even spot it.
After Care:
Results will indicate whether there is considerable acid reflux which needs treating. It was considered the "gold standard" for reflux but has been replaced in many hospitals by the more informative impedance study. this also measure pressure changes in the oesophagus and can usefully diagnose allergic reflux which is usually alkaline and would otherwise be missed on a ph study. It cannot show whether there is penetration of the airway, (aspiration) and whether surgery is required.
